Job summary

Harlem United Community Aids Center Inc in New York, NY is seeking a Social Worker who will provide client assessments, counseling, HIV education, and entitlement advocacy at the Adult Day Health Care Center.

Under supervision of the Managing Clinical Director, the Social Worker conducts group sessions, coordinates care, and ensures patient data confidentiality in the EHR; CASAC certification is a plus. Masters in social work with LMSW/LMHC preferred and housing/Medicaid entitlements valued.

Qualifications

• MSW or related graduate degree; LMSW/LMHC preferred.
• HIV/AIDS service experience preferred; group facilitation required.
• Excellent organization and communication skills.

Responsibilities

• Establish treatment goals within interdisciplinary care planning.
• Provide crisis intervention and refer for psychiatric assessment if needed.
• Coordinate multidisciplinary services influencing housing and entitlements.
• Maintain patient information confidentiality in the EHR.
• Participate in joint case conferences with other providers.
