Qualifications

• MSW or related graduate degree required.
• LMSW/LMHC preferred.
• Experience with HIV/AIDS in healthcare setting preferred.
• Experience with group facilitation required.
• Housing and Medicaid entitlements experience preferred.

Responsibilities

• Establish treatment goals and objectives with clients as part of the care plan.
• Provide crisis intervention services and refer for psychiatric assessment if needed.
• Coordinate multidisciplinary services per the interdisciplinary care plan.
• Identify clients served by other providers and coordinate care.
• Serve as primary social worker coordinating joint services and case conferences.
• Ensure the confidentiality of all patient information in the EHR.
• Lock the system or exit the application when away from the computer.
• Other duties as assigned by the Managing Clinical Director.
