Job summary

APLA Health seeks a PSS Social Worker to provide coordinated health care services for patients, with emphasis on engaging those facing psychosocial stressors like homelessness and mental health challenges. You will work in clinical settings or at patients’ homes, using a trauma-informed lens and linking to available resources.

Responsibilities include outreach, screenings, case planning, and ongoing assessment, with documentation in clinical systems and collaboration across care teams.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, or related field from an accredited program.
• Experience in care coordination or case management in clinical or community settings preferred.
• Experience with HIV/AIDS related care is a plus.

Responsibilities

• Help patients navigate APLA Health resources including benefits, housing, and HIV+ support services.
• Provide referrals, advocacy and service information to patients as needed.
• Use trauma-informed approach to support treatment engagement and retention; monitor progress toward goals.
