Job summary

Planned Parenthood of Greater New York is seeking a Community Outreach Social Worker to perform psychosocial assessments, develop individualized care plans, and coordinate care across internal and external partners. You will engage participants in community, mobile health, and clinical settings to improve access to care and reduce health disparities.

The role supports HIV prevention/treatment, PrEP/PEP, and GAHT referrals within licensure and program requirements.

Qualifications

• Master’s Degree in Social Work and NY LMSW license.
• Experience in behavioral health, HIV services, or care coordination.
• Ability to work independently in mobile community settings.

Responsibilities

• Perform psychosocial assessments and develop care plans.
• Coordinate services across departments and with external providers.
• Provide outreach, referrals, and linkage to care for HIV prevention and treatment.
