Job summary

The Department of Health and Social Services (DHSS) in Delaware is hiring a Health/Human Service Case Manager II to determine eligibility for SNAP, Medical Assistance, CHIP, and TANF, and to coordinate services with partners.

You will assess needs, develop service plans, monitor outcomes, document cases, and connect clients with housing, employment, and food resources in a trauma-informed, client-centered manner.

Qualifications

• Six months' experience in case management and ability to assess, plan, implement, monitor, and evaluate services.
• Six months' experience making recommendations as part of a client service plan (eligibility, treatment, counseling).
• Six months' experience interpreting laws, rules, and procedures.
• Six months' experience using automated information systems to enter and report data.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients and sources to determine needs for health and human services, including employment and skill development.
• Develops comprehensive service plans utilizing in-house sources, referrals, or providers.
• Conducts ongoing monitoring and assessment of service delivery and adjusts plans as needed.
• Maintains case records, summaries, reports, and required forms.
• Establishes effective relationships with clients and professional sources.
• Provides information and case coordination on-site, via email or telephone to individuals or groups.
• Assists with supportive casework before, during, or after hospitalization as needed.
• Uses information systems to research and report health/human service data.
