Job summary

Department of Health and Social Services in Delaware seeks a Health/Human Service Case Manager Supervisor to coordinate case management and training functions, supervise staff, and ensure program compliance with state and federal requirements.

The role involves planning, reviewing, and evaluating staff performance, coordinating with health and human service partners, and supporting grant writing and program evaluation activities as needed.

Qualifications

• Three years' experience in case management including assessment, planning, development, implementation, monitoring and evaluating options and services.
• Three years' experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
• Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
• Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
• Six months' experience in narrative report writing.

Responsibilities

• Supervises two or more merit full-time positions.
• Coordinates program planning, implementation, and coordination to ensure compliance with laws, rules and standards.
• Gathers statistics and information to monitor program performance and budget needs.
