Job summary

Department of Health and Social Services (DHSS) in Delaware seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for needs-based benefits and coordinate services for vulnerable residents.

You will work with TANF, SNAP vendors and other agencies, providing case management, referrals, and documentation while maintaining professional boundaries and compassionate service.

Qualifications

• Six months' experience in case management affecting eligibility for health or human services.
• Six months' experience interpreting laws, regulations, and procedures.
• Six months' experience using automated information systems for data entry and reporting.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients and sources to determine needs for health and human services.
• Develops comprehensive, cost-effective service plans using in-house or external providers.
• Monitors service delivery and authorizes changes for quality and efficiency.
• Maintains case records, reports, and documentation.
• Establishes relationships with clients, families, and community sources.
• Provides information and referrals to connect clients with needed services.
