Job summary

Health Connect America seeks a Foster Care Case Manager to coordinate services for children in the foster system. You will build individualized plans, coordinate multidisciplinary services, and maintain thorough case records to ensure consistent, family-centered care.

The role emphasizes collaboration with foster families, birth families, and external agencies, while meeting COA/state standards and supporting child safety and well-being.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104 and related options.
• Full-time supervised experience serving children/adolescents with primary mental health diagnosis.

Responsibilities

• Develop and implement individualized treatment and service plans.
• Coordinate medical, educational, behavioral health, and social services for seamless care.
• Serve as central point of communication between foster parents, families, and providers.
• Monitor progress and adjust care plans to evolving needs.
• Maintain accurate case records and documentation.
