Job summary

Health Connect America, Pinnacle Family Services is seeking a Foster Care Case Manager to support children and families in the foster care system across North Carolina. You will coordinate services, develop treatment plans, and communicate with foster parents and agencies to ensure safe, stable, and individualized care.

You will also monitor progress, maintain records, and collaborate with internal teams to meet regulatory standards and outcomes, while benefiting from a robust benefits package

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• One of the following: Master’s in human services field + 1 year experience; Bachelor’s in human services field + 2 years experience; Bachelor’s in non-human services field + 4 years experience; Current NC license or board-issued certification in a human services profession.

Responsibilities

• Develop and implement individualized treatment and service plans that support each child’s safety, stability, and well-being.
• Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care.
• Serve as a central point of communication between foster parents, biological families, service providers, and internal teams.
• Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes.
• Monitor and continuously assess each child’s progress, adjusting care plans to meet evolving needs.
• Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards.
• Partner with foster parents to provide guidance, support, and resources that help create safe, nurturing home environments.
• Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements.
