Job summary

StepStone Family & Youth Services seeks a compassionate Foster Care Case Manager to support children, youth, and foster families through trauma-informed, strengths-based, and family-centered practices. You will coordinate services, advocate, and maintain accurate documentation to promote safety, stability, and permanency.

The role involves coordinating with foster parents, biological families, schools, referral agencies, and treatment teams to ensure resources are available for positive youth

Qualifications

• Bachelor's degree in a human services field or nursing field.
• Valid driver's license.
• Two years of experience in case management or related discipline.
• Must be able to communicate both verbally and in writing.

Responsibilities

• Receives and responds to incoming referrals and assesses client needs with the clinical team.
• Completes pre-screening assessments and related paperwork to initiate services.
• Oversees and guides field staff involved in the pre-screening process.
• Maintains knowledge of services and resources to support placement stability.
• Documents accurately and ensures HIPAA compliance and program policies.
