Job summary

Community Action Partnerships of Western Nebraska (CAPWN) is seeking a dedicated Family Stabilization Case Manager. You will implement emergency assistance and case management activities, perform home visits when needed, and help clients develop goal setting and action plans.

You will advocate for clients, coordinate CAPWN services with local resources, assess eligibility for programs, maintain confidentiality, and deliver trauma-informed support.

Qualifications

• 1 year experience in a human service program or agency working with clients facing insufficiencies.
• Computer experience required.
• Proficient in verbal communication and in maintaining written documentation.
• Bilingual preferred, incentive pay available.
• Age 21+ preferred
• Valid driver’s license with good driving record required and insured vehicle available is preferred.

Responsibilities

• Implement the Family Stabilization (emergency assistance) and case management programs; home visits when needed; assist clients with the development of goal setting and action plans; advocate for clients; coordination of CAPWN services and other area resources; assessment of clients for eligibility of services for all programs, occasional assist with Special Distribution projects.
• Complete initial client interview, required paperwork, and intake in agency data tracking system for emergency assistance.
• Assess, and assist families in the development of a self-sufficiency plan and track client progress in agency database.
• Work in conjunction with Family Stabilization Committee in determining client eligibility for assistance requests.
• Maintain confidentiality of client information and provide a supportive and Trauma informed environment for all client interactions.
• Adhere to all funding source guidelines.
• Make referrals to outside resources as needed.
• Provide case management support and assist families in meeting their goals.
• Coordinate with CAPWN programs and area agencies to recruit and coordinate services for client families.
• Provide training sessions for clients to develop self-sufficiency and life skills.
• Provide outreach services.
• Maintain confidentiality of records and communications following CAPWN policy and HIPAA privacy and security policies and procedures.
• Complete required reports for program and agency paperwork.
• Attend meetings and trainings as required.
