Job summary

Delta-T Group, Inc. is seeking a Family Empowerment Case Manager for temporary assignments in the Northern Liberties area of Philadelphia.

The contractor will conduct comprehensive intake assessments, manage a caseload of up to 15 families, and work 9am–5pm, Monday through Friday, under Family Empowerment Services. The role requires a bachelor's degree in social work, psychology, or related fields, plus trauma-related experience, clearances, CPR/First Aid, and health documentation.

Qualifications

• Bachelor's degree in social work, psychology, human services, sociology.
• Valid Driver's License.
• 1 year experience working with populations affected by traumatic experiences.
• 1 year experience in human services field as case manager, service coordinator, or social worker.
• Clearances: FBI, Child Abuse, State Background and ACT 31.
• Produce current medical/health documents including: physical examination, TB test, CPR, and First Aid Level 2 certification, & hepatitis B screening.
• Adhere precisely to interventions in treatment plan(s).

Responsibilities

• Interact with families who have accepted FES and conduct comprehensive intake assessments.
• Must have experience dealing with trauma and crisis situations.
• Responsible for engagement process with families referred to FES.
• Responsible for timely documentation and accurate daily data collection.
• Maintain caseload of up to 15 families.
• Ability to be at assignment 9am-5pm Monday through Friday.
• Other duties as deemed appropriate by the supervisor/manager
