Qualifications

• Master’s degree in Social Work, Counseling, or a related field.
• Current clinical license in Texas (LCSW, LPC, or equivalent).
• 3–5 years in case management, behavioral health, or supportive housing with supervisory experience.
• Trauma-informed, culturally responsive, and person-centered practices.
• Ability to supervise and develop staff in a team-based environment.
• Strong data analysis, program evaluation, and quality-improvement skills.
• Excellent communication and relationship-building with partners.
• Travel/commute to Foundation Communities properties as needed.
• Maintain professional boundaries and confidentiality.
• Flexible schedule including occasional evenings or weekends.

Responsibilities

• Foster a trauma-informed work environment that supports staff well-being and resilience.
• Coordinate and monitor contracted therapeutic providers for quality and alignment.
• Track and analyze program data to monitor outcomes and utilization.
• Prepare reports showing impact, trends, and guide CQI efforts.
• Collaborate with property management and Learning Center leadership for coordinated resident support.
• Build partnerships with behavioral health providers and community organizations.
• Represent Foundation Communities at meetings, coalitions, and events related to resident services.
• Support outreach and participate in leadership meetings and cross-departmental initiatives.
• Uphold equity, inclusion, and trauma-informed care in service delivery.
• Perform additional duties to support program and agency goals.
• Provide leadership and supervision for CSS Case Management, including Clinical, Development, and General Case Managers.
• Supervise, coach, and evaluate staff performance with reflective supervision and development opportunities.
