Job summary

JEVS is seeking a START Coordinator to join a clinical team delivering community-based assessment, crisis prevention, planning, and therapeutic coordination for individuals with IDD and behavioral health needs.

You will manage a caseload, develop CSCPIPs, coordinate supports, and work across systems to promote integrated care, with on-call crisis response and commitment to trauma-informed START practices.

Responsibilities

• Manage a caseload of 20-30 individuals with dual diagnoses (IDD and behavioral/mental health needs).
• Develop individualized Cross-Systems Crisis Prevention and Intervention Plans (CSCPIPs).
• Conduct assessments and interventions in collaboration with the clinical team and natural support systems.
