Job summary

Horizon Health and Wellness, Inc. seeks an Outreach Specialist to engage and re-engage community members into primary care, mental health, and substance use services. The role emphasizes trauma-informed recovery and documenting services to support treatment plans.

The position requires a valid driver’s license, ability to lift 50 pounds, and proficiency with electronic health records; bilingual English/Spanish is preferred.

Qualifications

• High School diploma or GED.
• Minimum of two years in behavioral health or related field.
• Must be 21 years or older with a valid driver’s license and reliable transportation.
• Certification in first aid, CPR, and crisis de-escalation (training provided if not active).
• Ability to lift up to 50 pounds.

Responsibilities

• Engage and re-engage community members into Primary Care, Mental Health, or Substance Use Treatment services.
• Provide outreach services in a Trauma-Informed Recovery Focused manner as per individual treatment plans.
• Document services provided and create a positive experience for individuals served; maintain professional conduct with Kindness Matters.
