Job summary

Horizon Health and Wellness, Inc. is seeking a Crisis Outreach Care Manager to facilitate enrollment and discharge, conduct follow ups, and deliver crisis intervention across age groups.

This role emphasizes community outreach, education, and advocacy within central Arizona communities. The candidate should have a High School Diploma or GED with 4 years of behavioral health experience; First Aid/CPR and CPI; a valid driver’s license, and a flexible schedule including weekends and holidays.

Qualifications

• High School Diploma or GED plus 4 years’ experience in behavioral health or related education/experience to meet Behavioral Health Technician qualifications.
• First Aid, CPR and CPI training certification; agency will provide if not possessed.
• Must be at least 21 years of age and have a valid driver’s license with acceptable driving record.

Responsibilities

• Facilitate enrollment and discharge, conduct follow ups, and provide crisis intervention for all age groups.
• Respond to crisis calls in person within 30 minutes or by phone within 15 minutes; address emergencies calmly and promptly.
• Spend at least 50% of time meeting community members and stakeholders; educate about entitlements and behavioral health services; provide consultation, education and advocacy.
