Job summary

Qualifications

• Must have a Master’s or higher in a mental health field or a Bachelor’s with WI CSW license.
• Must be self-motivated, excited, and passionate about mental health and substance use.
• Willingness to meet clients in homes, community, or office.
• Maintain licensure requirements and complete required training within 60 days.

Responsibilities

• Perform intake assessments and comprehensive life domains within set timelines.
• Develop 6-month Recovery/Treatment Plans by day 27 and update at least every six months.
• Complete clinical documentation within required timeframes per CCS/Medicaid standards.
• Provide case management, advocacy, and linkage to services (housing, medical, legal, financial).
• Engage in regular supervision and professional development; support travel to Dane County communities.
