Job summary

Community Living Alliance is hiring a Full Time Service Facilitator to support recovery for individuals at multiple sites in Dane County. Work involves client homes, offices, and potential remote tasks after competency is shown.

Schedule is Mon-Fri, 8:30AM-4:30PM with no weekends; compensation is $23-25 per hour depending on education and experience. Benefits include health, dental, vision, and PSLF, plus mileage reimbursement and growth opportunities.

Qualifications

• Six (6) months experience working with individuals with mental health and/or substance use conditions, intellectual or physical disabilities, chronic health conditions or seniors.
• Six (6) months experience providing psychosocial rehabilitation services to individuals with mental health and/or substance abuse diagnosis, required.
• Valid driver’s license, reliable transportation, and current auto insurance to travel throughout Dane County.
• Pass a WI Caregiver Background Check.
• Must be authorized to work in the United States.

Responsibilities

• Provide support for individuals working toward recovery using a supportive, strengths-based, and culturally humble approach to treatment.
• Facilitate and coordinate services relevant to individuals’ recovery plans and community support needs.
• Utilize effective strategies to engage program participants, enhance motivation, and encourage meaningful, collaborative, and strength’s-based goal-planning and progress.
• Provide screening and assessments, service linkage, service facilitation and other allowable and appropriate services on the CCS service array in a trauma-informed, culturally relevant and non-stigmatizing manner.
• Assist the program participant with identifying and establishing natural and professional supports in the community.
• Maintain contact with program participants to support effective engagement and monitor progress.

Education

Bachelor/Master’s Degree in Mental Health
