Job summary

Felton Institute is seeking a Clinical Case Manager for Felton Outpatient Services Solano (FOSS) Adult Division. The role emphasizes trauma-informed care, interdisciplinary collaboration, and community outreach to support adults with complex mental health needs in Solano County.

The position requires a Master’s degree and CA licensure with strong documentation skills; it includes local travel between sites and onsite work schedule.

Qualifications

• Master's degree in a relevant field and CA licensure or under supervision.
• 2 years of experience in mental health, homelessness, and related care preferred.
• Proficient with EHR and charting under payors; strong DSM knowledge.

Responsibilities

• Provide trauma-informed, culturally responsive mental health services.
• Coordinate care and collaborate with medical and community partners.
• Engage in outreach to shelters, hospitals, and community settings.
• Document all services timely and accurately in charts.
• Offer on-call triage during working hours and team coverage.
• Attend trainings and participate in program development.
