Job summary

The San Diego LGBTQ Community Center is seeking a Case Manager to support housing stability through trauma-informed, wrap-around services. You will conduct screenings, connect clients with mental health, substance use, education, and career resources, and work within a collaborative team.

On-site at our Hillcrest office, you will document progress, maintain records in HMIS, and help prepare monthly funder reports while upholding equity and client-centered care.

Qualifications

  • 1 year of experience providing case management services to people experiencing homelessness or at risk of homelessness.
  • Fluency in English and Spanish required.
  • Proficiency with Microsoft Office and PC-based environments.
  • Ability to manage multiple tasks with shifting priorities.

Responsibilities

  • Conduct intakes, assessments, and evaluations with new clients.
  • Provide links to mental health services, substance abuse services, educational services, career development services, and other relevant resources.
  • Provide ongoing case management, crisis intervention, conflict resolution, and stabilization services to participants enrolled in the program.
  • Complete individualized service plans and self-sufficiency tools.

Position Summary

The Case Manager is responsible for providing case planning, implementing outreach to eligible participants, conducting assessments, and crisis intervention to participants of The Center's expanding LGBTQ affirming wrap around and support services. The case manager works collaboratively with a team in a trauma-informed, and goal-oriented way to help resolve the housing stability of program participants.


Primary Roles & Responsibilities

Client Support


  • Conduct intakes, assessments, and evaluations with new clients.

  • Provide links to mental health services, substance abuse services, educational services, career development services, and other relevant resources.

  • Provide ongoing case management, crisis intervention, conflict resolution, and stabilization services to participants enrolled in the program.


Record Management And Reporting


  • Complete individualized service plans and self-sufficiency tools.

  • Maintain accurate documentation of client's goals and needs, case management plans, and service entries into internal data systems.

  • Enter program data and outcomes into the region's Homeless Management Information System (HMIS) to adhere to grant and contract compliance.

  • Document thorough case notes that accurately reflect client progress toward individualized case management goals.

  • Assist in the completion of monthly narrative reports for funder reports.

Required Qualifications


  • Education and/or Experience: 1 year of experience providing case management services to people experiencing homelessness or at risk of homelessness.

  • Language Skills: Fluency in English and Spanish required.

  • Computer/Office Equipment Skills: Must be comfortable working in PC/Windows-based environments. Must have strong proficiency with Microsoft Office.

  • Other Qualifications:

  • Demonstrated ability to manage multiple tasks with shifting priorities.

  • Must demonstrate competence in using strength-based, trauma-informed, and Motivational Interviewing Techniques.

  • Reliable transportation, a valid driver's license, and auto insurance.


Preferred Qualifications


  • Education: Bachelor's degree in a social service-related field.

  • Experience: 2 or more years of experience providing case management services to people experiencing homelessness or at risk of homelessness.

  • Other Qualifications: Knowledgeable about current issues related to the LGBTQIA+ community and people experiencing homelessness.


WORK LOCATION


  • On- site, in the office and with program participants in the field.

  • Office in the Hillcrest Community in the City of San Diego.


Compensation & Benefits

Compensation: The pay range for this position is $23.00/hour- $25.00/hour and is commensurate with experience. This is a Full-Time, Regular Non-Exempt position.
