Responsibilities

• Conduct outreach and assertive case management for trauma survivors to access services.
• Coordinate service plans with agencies, clinicians, and local health partners.
• Assist clients in setting and achieving personal goals and navigating resources.
• Maintain and document case records and prepare reports as required.
• Advocate for clients’ rights and serve as liaison between patients and providers.

Education
