Bridges To Change Inc in Wisconsin/Oregon is seeking a full-time Case Manager I to support male-identifying participants with recovery goals, community integration, and access to essential resources. The role emphasizes trauma-informed engagement, documentation, and collaborative care coordination.

Responsibilities include direct client services, skills development, transportation assistance, and crisis response within a respectful, confidential, and culturally responsive framework.

Qualifications

• Minimum 1 year of continuous sobriety for those with addiction lived experience.
• Peer Wellness Specialist (PWS) certification required within 90 days of hire.
• Valid driver’s license and reliable vehicle for travel.
• Background check clearance as required by program regulations.

Responsibilities

• Deliver peer support services and case management with at least 50% direct client contact.
• Assist participants in accessing community resources and supports.
• Document services, progress notes, and interactions within 24–72 hours.
• Provide trauma-informed counseling, de-escalation, and safety planning as needed.
