Omni Family of Services Tennessee Inc. seeks a Care Coordinator to meet with clients and their families in their homes and other locations to assess needs and coordinate services.

The role involves diverse settings (school, court, job site) and may include on-call responsibilities to support clients. Hours vary with caseload and may approach 40 hours per week, with an emphasis on trauma-responsive, ESTEE M values and collaborative care.

Qualifications

• Bachelor's degree in a behavioral health related field (counseling, social work, or psychology preferred).

Responsibilities

• Meet with clients and family in home or residence to assess needs and plan services.
• Coordinate scheduling of services and face-to-face and phone triage assessments.
• Be available for on-call or after-hours support as needed within caseload requirements.
• Collaborate with schools, court, job sites, and community supports to meet client goals.
