Job summary

LifePath Systems in McKinney, TX seeks a Care Coordinator within the Behavioral Health division to provide care coordination and outpatient behavioral health services across the LifePath system of care.

You will maintain a caseload, use initiative to connect individuals with timely, quality care, and work with LifePath teams and community partners to remove barriers and ensure smooth transitions across health and social services.

Qualifications

• Bachelor's degree in psychology, social work, or related field.
• CCBHC knowledge and experience required.
• Experience in Texas Recovery and Resiliency (TRR) required.
• Knowledge of Trauma-Informed Care Practices required.
• Must have reliable transportation and a valid driver's license.

Responsibilities

• Perform care coordination and outpatient behavioral health services.
• Maintain a caseload and ensure timely access to quality care.
• Identify needs and reduce barriers to care through community collaboration.
• Provide referrals internally and externally as needed to meet needs.
• Ensure gaps in care are reconciled within a trauma-informed, person-centered model.
• Uphold professional conduct and stewardship of resources.
