Open Sky Community Services seeks a Coaching & College Navigation Clinician to provide person-centered, clinically informed coaching, college navigation, and case management for young adults and adults with Autism and other developmental needs in central Massachusetts. Services span homes, community sites, campuses, agency locations, and virtual formats.

Qualificações

• Master’s degree in social work, psychology, counseling, or related clinical field is required.
• Minimum of three years of experience supporting individuals with developmental disabilities, Autism, and mental health challenges.
• Familiarity with Positive Behavioral Supports, trauma-informed practice, and person-centered planning.

Responsabilidades

• Engagement, assessment, and person-centered planning with individuals.
• Provide individualized coaching in homes, community settings, campuses, and virtual environments.
• Clinical coordination including care management, referrals, and safety planning.
• Documentation, outcomes tracking, and quality improvement activities.
