Job summary

EmberHope Connections, LLC seeks an Aftercare Specialist in Wichita, KS to support families exiting child welfare services. You will conduct weekly contacts, coordinate meetings with families and case managers, and help develop aftercare plans that promote stability and wellbeing.

The role requires a 4-year degree, 1–3 years in child welfare preferred, and the ability to travel and work flexible hours, including evenings and weekends.

Qualifications

• Bachelor's degree in social services or related field.
• 1–3 years of child welfare experience preferred.
• 21 years of age; must pass background check, fingerprinting, and screenings.
• Maintain company-required trainings.
• Physical working requirements - ability to travel, drive company or personal vehicle including at night; up to 50 lbs; stairs; flexible schedule.

Responsibilities

• Ensure a trauma-informed focus on safety and resilience for families.
• Provide weekly phone and monthly in-person contacts with all family members; coordinate meetings.
• Collaborate with case managers and facilitate case transfers with DCF as needed.
• Engage families throughout the aftercare period.
• Create and monitor an Aftercare Plan with the family and team.
• Help families access community resources and APPLA supports.
• Follow EmberHope policies and procedures; adhere to Program Practice Guidelines.
• Back up on-call to the on-call department.
