2INgage is seeking an Aftercare Case Manager in Independence, KS. You will support children and families achieving permanency through reintegration, adoption, guardianship, or aging out of care, with a focus on stability and crisis intervention.

You’ll collaborate with agencies, maintain documentation, and coordinate community services to support reunification. A Bachelor’s degree in human services and a valid license are required, plus training will be provided.

Formación

• Bachelor’s degree in human services required.
• Valid driver’s license required.
• Willingness to learn and complete training provided.

Responsabilidades

• Support children and families achieving permanency through reintegration, adoption, guardianship, or aging out of care.
• Focus on risk management to help families maintain stability during the aftercare period.
• Provide crisis intervention and coordinate with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families; meet families in person to support reunification.
