Job summary

TFI Family Services in Parsons, KS is seeking an Aftercare Case Manager to support children and families who have achieved permanency. The role includes crisis intervention, coordinating services, and in-person meetings to support reunification.

You’ll work full-time with a focus on risk management, documentation, and collaboration with community resources. A bachelor’s degree in human services and a valid driver’s license are required, with training provided.

Responsibilities

• Support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.
• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve concerns.
