Centerstone's Access Center Case Manager provides assessment and services to a variety of clients in need of referrals to inpatient care or diversion. This position provides services within Centerstone's Behavioral Health Hospital in Bradenton. This role is responsible for conducting intake assessments, coordinating admissions and discharges, and connecting clients with appropriate levels of care. Working with individuals of all ages, including those involved in Baker Act and Marchman Act cases, this position plays an essential role in crisis response, care coordination, and ensuring clients receive the support and services needed throughout their treatment journey. This position will work a full-time overnight schedule. 6:00 pm - 6:00 am Wednesday, Thursday, Friday and every other Saturday.
Valid Florida Driver's License with a safe driving record and current auto insurance required.
