Centerstone is seeking a Case Manager for our Inpatient Behavioral Health Hospital in Florida. You will provide individual and group counseling, coordinate services, and link clients to community resources to support recovery.
The role requires collaboration with local partners, developing treatment plans, and maintaining timely EMR documentation. Schedule is Monday–Friday, 9:00 am–5:00 pm with opportunities for additional training and certification.

Provide mental health, substance abuse, and outreach services to individuals who may have a mental health and/or substance use disorder within Centerstone’s Inpatient Behavioral Health Hospital. This role will collaborate with community partners and local stakeholders to provide comprehensive, person-centered care. The Case Manager will meet with individuals across several units within the inpatient facility. Through case management services, this role will connect clients and their families with resources that support recovery and improve quality of life. A variety of clinical training opportunities are available based on program needs and individual interests.
Schedule: Monday - Friday 9:00 am - 5:00 pm
Bachelor’s degree required. Good organizational, time management, written and verbal communication skills. Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries.
Community-based program case managers, who either take crisis calls and/or home visits, must have a valid FL driver’s license with a good driving record and current auto insurance.
