San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is an innovative program for individuals aged 55 or older with chronic illnesses or disabilities who need coordinated medical care to maintain independence at home and in the community. The healthcare teams at St. Paul’s PACE provide a hands-on approach, coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

Basic Function: The PACE Transportation Coordinator is responsible for coordinating and scheduling transportation for PACE participants. Ensures clear communication and that drivers follow directions. Organizes routes and schedules daily transportation to and from the PACE Center and medical appointments.

Essential Duties and Responsibilities:

  • Attend IDT and POC daily meetings.
  • Schedule transportation for all PACE participants and respond to needs identified by the Interdisciplinary Care Team.
  • Coordinate transportation to ensure timeliness and operational efficiency.
  • Adjust schedules as needed, communicate changes to relevant personnel.
  • Review and audit daily trips, and prepare reports.
  • Develop positive relationships with PACE participants, families, caregivers, colleagues, and staff, promoting excellent customer service.
  • Maintain transportation records and ensure policies and procedures are followed.
  • Respond to needs identified by the Care Team and communicate adjustments.
  • Report issues promptly with possible solutions to management.
  • Perform other duties as assigned.

Education & Experience:
