A leading care center in Auburn seeks a dedicated Social Services Assistant to support the Social Service Director in enhancing resident care. The role involves planning and implementing social service programs, assisting residents with their needs, and ensuring compliance with policies. Description
Assist the Social Service Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility. Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed. Assist in the development, administering, and coordinating of department policies and procedures. Review department policies and procedures at least annually, and participate in making recommended changes. Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.
Participate in community planning related to the interests of the facility and the services and needs of the resident and family. Assist residents in achieving the highest practicable level of self-care, independence, and well-being. Assist with providing medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
Perform administrative requirements, such as completing necessary forms and reports, and submitting them to the Director. Provide information to residents and families regarding Medicare/Medicaid and other financial assistance programs. Provide consultation to staff and community agencies to develop social service programs addressing resident needs.
Record and maintain social service progress notes, coordinate social service activities with other departments, and work with emotional problems including assisting residents and families with anxieties, stress, residual disabilities, fears, and the need for specialized care. Evaluate social and family information to determine social treatment plans.
Meet regularly with department personnel to identify and correct problems or improve services. Obtain resources from community social, health, and welfare agencies. Assist with outpatient appointments and ancillary services such as optometry, podiatry, dentistry, and psychiatry. Assist with discharge planning, develop good rapport with other departments and outside agencies, and participate in orientation and training programs.
Assist in developing and managing the department's budget, report suspected fraud, and maintain confidentiality of all resident care information, including protected health information.
Supervisory Requirements: This position has no supervisory responsibilities.
Qualifications: Must possess a high school diploma or equivalent; Associate or Bachelor's Degree preferred. Prior healthcare experience is preferred.
Skills: Ability to read, analyze, interpret, write reports, and present information effectively. Basic math skills and problem-solving abilities are required.
Physical Demands: Occasionally lift/move up to 25 pounds, use of computer, and assistance in emergency evacuations may be required.
Work Environment: Low to moderate noise level, reasonable accommodations provided for disabilities.
