Job summary

Chamberlin Healthcare Center, part of the CommuniCare Family of Companies, seeks a Transportation Assistant to safely escort residents to outside physician visits and in-house appointments.

Responsibilities include transporting residents within the facility, monitoring elopement risks, and maintaining records. A high school diploma, valid driver’s license, Nurse Aide training (or eligibility), and Red Cross First Aid are required.

Qualifications

  • High school diploma or GED required.
  • Must have a valid driver’s license in the state of employment.
  • Graduate of an approved Nurse Aide Training Program or eligible and must graduate and pass state test within 120 days.

Responsibilities

  • Transport and assist residents to and from outside physician visits in a safe manner.
  • Assist with transporting residents within the facility to scheduled appointments and activities.
  • Monitor residents deemed elopement risk during outside visits.
  • Provide basic nursing care procedures as required.
  • Maintain resident documentation records and adhere to policies and safety standards.

This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.****JOB DUTIES & RESPONSIBILITIES***** Transport and assist residents, per facility vehicle, to outside physician visits in a safe manner.* Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.* Assist with transporting residents to and from scheduled in-house activities.* Provide one on one monitoring for residents deemed an elopement risk on outside visits.* Perform basic nursing care procedures for residents as required.* Create a clean, comfortable, and safe environment for residents and assure that their needs are met.* Maintain resident documentation records.* Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.* Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.* Perform other related activities as assigned or requested.****QUALIFICATIONS & EXPERIENCE REQUIREMENTS***** High school graduate or G. E.
