Maplewood Healthcare Center in Bridgeport, WV, is seeking a Transportation Assistant to safely move residents to outside medical appointments and within the facility. The role may transition to Activities/CNA duties as needed and requires a WV CNA license and a WV driver’s license.

Responsibilities include monitoring elopement risks, basic nursing tasks, and maintaining records while following facility policies and participating in ongoing training.

Qualifications

• High school diploma or GED required.
• Valid WV Driver's License and WV CNA license required.
• Graduate of approved Nurse Aide Training Program or eligible to enter and complete within 120 days.
• Red Cross First Aid Certificate required.
• Independent decision-making experiences preferred.
• Healthcare or service industry background advantageous.

Responsibilities

• Transport residents to outside physician visits safely.
• Assist in Activities and perform CNA duties when needed.
• Transport within facility to scheduled specialty appointments.
• Transport residents to in-house activities.
• Provide monitoring for elopement risk during outside visits.
• Perform basic nursing care procedures as required.
• Maintain resident documentation and adhere to policies.
• Attend trainings and maintain certifications.

Job Address:1081 Maplewood Drive Bridgeport, WV 26330Maplewood Healthcare Center, a member of the CommuniCare Family of Companies is currently recruiting for a Transportation Assistant to join our team.Primarily Dayshift, Monday - Friday with weekends as needed.PURPOSE/BELIEF STATEMENT:The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures. This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.JOB DUTIES & RESPONSIBILITIESTransport and assist residents, per facility vehicle, to outside physician visits in a safe manner.When not working in transportation, will be expected to work in Activities department as well as working as a CNA if needed.Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.Assist with transporting residents to and from scheduled in-house activities.Provide one on one monitoring for residents deemed an elopement risk on outside visits.Perform basic nursing care procedures for residents as required.Create a clean, comfortable, and safe environment for residents and assure that their needs are met.Maintain resident documentation records.Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.Perform other related activities as assigned or requested.QUALIFICATIONS & EXPERIENCE REQUIREMENTSHigh school graduate or G.
