Job summary

Clay Healthcare Center is seeking a part-time Transportation Assistant in West Virginia to help residents travel to doctor visits and on-site activities. You will ensure safety, monitor elopement risks, and support the care team with basic nursing tasks as needed.

Responsibilities

• Transport and assist residents via facility vehicle to outside physician visits safely.
• Assist with transporting residents within the facility to scheduled appointments and activities.
• Provide one-on-one monitoring for elopement-risk residents during outside visits.
• Perform basic nursing care procedures as required.
• Maintain a clean, safe, and comfortable environment and resident documentation.
• Adhere to policies, procedures, safety, and infection-control standards; participate in training.

**Clay Healthcare Center**, a member of the **CommuniCare Family of Companies** is currently recruiting for a **Transportation Assistant** to join our team.**Part Time Position Available****Day Shift Position Available****Rate $8-15/hour****PURPOSE/BELIEF STATEMENT:**The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures. This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.**JOB DUTIES & RESPONSIBILITIES*** Transport and assist residents, per facility vehicle, to outside physician visits in a safe manner.* Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.* Assist with transporting residents to and from scheduled in-house activities.* Provide one on one monitoring for residents deemed an elopement risk on outside visits.* Perform basic nursing care procedures for residents as required.* Create a clean, comfortable, and safe environment for residents and assure that their needs are met.* Maintain resident documentation records.* Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.* Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.* Perform other related activities as assigned or requested.**QUALIFICATIONS & EXPERIENCE REQUIREMENTS*** High school graduate or G. E.
