Merakey AVS is seeking a Licensed Practical Nurse to join our Crafton, PA location as a Transport Nurse. You will accompany clients to hospitals, providers, and on trips, while delivering essential personal care and support for adults with complex needs.

The ideal candidate holds a PA LPN license, with a preference for 1 year of LPN experience and a valid PA driver’s license. This role operates in a 45–50 bed setting within a PHMC facility, offering competitive hourly pay with benefits.

• Current LPN license in PA (or Commonwealth).
• One year of LPN experience preferred.
• Driver's license and reliable transportation may be required.

Responsibilities

• Accompany medical necessary clients to hospitals, outside providers, and on trips.
• Provide personal care, communication and social skill support.
• Assist and train individuals in health safety and daily living activities.

Job description

We are seeking a Licensed Practical Nurse to join our team in ourCrafton, PA location. This nurse will act as the Transport Nurse and will accompany medical necessary clients to hospitals, outside providers and on trips and outings.

Earn

$29 per hour with benefits or $31 per hour with the selection of enhanced pay and scaled back benefits.

Schedule:

M - F 8:30 AM - 9:00 PM

In addition, most of the people we serve have multiple physical disabilities, extensive medical complications and may require supports for challenging behaviors.This position is located in our PHMC facility that operates as a 45-50 bed location.

Merakey AVS is a non-profit organization which provides homes and a full range of services to adults with all levels of intellectual and developmental disabilities (IDD). The majority of the nearly 800 individuals that we support are diagnosed with severe or profound IDD.
