Merakey AVS is seeking an LPN Case Manager to join our Crafton, PA team. The role coordinates medical care for a designated caseload of residents and works with the health care team to implement the plan.

We offer pay at $29 per hour with benefits or $31 per hour with enhanced pay and scaled back benefits, and a schedule from Monday to Friday, 8:30 AM to 9:00 PM. We are a nonprofit provider serving adults with IDD, emphasizing comprehensive benefits and work-life balance.

Qualificações

• Current license as an LPN in the Commonwealth of PA.
• Preferred two years of hospital experience.
• Genuine interest in working with individuals with physical and developmental disabilities.

We are seeking an LPN Case Manager to join our team in ourCrafton, PAlocation.

Earn $29 per hour with benefits or $31 per hour with the selection of enhanced pay and scaled back benefits.

Schedule: M - F 8:30 AM - 9:00 PM

Requirements
