Qualifications

Responsibilities

• Conducts comprehensive assessments of patient and donor psychosocial, cultural, and spiritual needs.
• Collaborates with the transplant team to develop and implement individualized care plans.
• Provides counseling and support to patients and donors, facilitating decision‑making and addressing concerns.
• Participates in multidisciplinary rounds and transplant team meetings to discuss progress and plans.
• Refers patients/donors for additional evaluations and coordinates discharge planning and post‑transplant resources.
