Job summary

CARIN Alliance is seeking a Transplant Social Worker to support kidney transplant recipients and living donors throughout their journey. You will apply advanced social work practices, perform psychosocial assessments, and collaborate with an interdisciplinary team to address social, emotional, and economic needs.

You will respond to suspected abuse or neglect per ethical guidelines and laws, provide counseling and resources to patients, families, and caregivers, and support ongoing care planning

Qualifications

• Master’s degree in Social Work from an accredited program.
• Licensure in good standing (LMSW or LCSW) in the state of practice.
• Preferred healthcare and mental health hospital experience.

Responsibilities

• Applying advanced social work practices with expert critical thinking and psychosocial assessments.
• Responding to suspected abuse, neglect, or violence following ethical guidelines and laws.
• Providing specialized counseling and support to patients, families, and caregivers.
• Collaborating within an interdisciplinary team to address social, emotional, and economic concerns.
• Utilizing age-specific care protocols across diverse patient populations.
• Assisting with problem management related to illness impact and facilitating continued care planning.
