Job summary

Providence Swedish Medical Center in Seattle is seeking a Social Worker specializing in transplant psychosocial care to join the Organ Transplant team. You will assess potential living donors, provide emotional support, and coordinate financial referrals and services for patients and families.

In this role you will collaborate with inpatient and outpatient teams, supporting patients through pre- and post-transplant care and helping navigate complex care pathways in a patient-centered environment.

Qualifications

• Master's Degree in Social Work from an accredited school.
• Upon hire: Washington Clinical Independent Social Worker Associate License.
• Experience working with issues of chronic illness and disability.

Responsibilities

• Conducts psychosocial assessment of potential organ transplant living donors.
• Provides emotional support, financial referral, and concrete services to Organ Transplant patients and their families.
• Works as an integral part of the multidisciplinary Organ Transplant team in the inpatient and outpatient settings, pre- and post-transplant.
