Job summary

Montefiore Medical Center is seeking a Social Worker MSW to join the Moses Campus transplant unit, providing direct and indirect services for patients and families to support safe and timely discharge and treatment plans.

Responsibilities include psychosocial assessments, coordination of services, and advocacy across interdisciplinary teams, with emphasis on ethical and legal considerations in end-of-life care and patient safety.

Qualifications

• Master's Degree in Social Work from a school of Social Work accredited by the CSWE.
• LMSW or LCSW certification is required.

Responsibilities

• Assist patients/families and support systems to cope with the impact of illness and ensure safe, timely discharge/treatment plans.
• Screen patients for social service interventions, assess psychosocial needs, and ensure continuity of care.
• Implement and validate discharge/treatment plans to support patient recovery.
• Coordinate services and provide psychosocial education and feedback to patients and families.
• Refer clinically complex cases to Licensed Social Workers as appropriate.
• Organize workload with appropriate parties based on medical plans and patient needs.
• Demonstrate proactive advocacy and negotiation with patients, families, associates and community.
• Maintain professional conduct and participate in internal/external conferences.
• Utilize psychological knowledge to assist with team building, discharge/treatment planning and education of staff.
• Demonstrate knowledge of legal and ethical issues in end-of-life care, including informed consent and advance directives.
• Identify, report and intervene for victims of abuse and neglect.
