Responsibilities

• Assist patients / families and support systems to cope with illness and facilitate discharge/treatment plans.
• Screen patients for social service interventions and assess psychological needs.
• Coordinate services and provide education and referrals.
• Refer complex cases to Licensed Social Worker and ensure continuity of care.
