Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.
• ADN required, BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborate to enhance delivery of patient care along the continuum.
• Meet patient needs efficiently and expeditiously by improving patient experience.
• Evaluate, predict, and facilitate the trajectory of patient care.
• Assess, plan, implement, care coordination, monitoring, advocacy and evaluation.
