Job summary

Montefiore Medical Center is seeking an Ambulatory Clinical Pharmacy Manager for Solid Organ Transplantation to lead outpatient transplant pharmacotherapy services. Qualifications

• Doctor of Pharmacy degree and NYS license to practice pharmacy.
• Completion of PGY2 Residency in Solid Organ Transplant highly preferred.
• Board certification in Solid Organ Transplant (BCTXP) preferred.
• Prior leadership or supervisory experience is preferred.

Responsibilities

• Provide clinical pharmacy leadership for outpatient solid organ transplant programs.
• Oversee development and maintenance of CDTM protocols and clinical practice guidelines.
• Direct patient care including immunosuppression management and therapeutic drug monitoring.
• Collaborate with transplant physicians and pharmacists to optimize patient outcomes and ensure regulatory compliance.
