Qualifications

• Doctor of Pharmacy from ACPE-accredited college of pharmacy.
• Current NYS pharmacist license/registration.
• PGY2 Residency in transplant preferred.
• Board certification in transplant preferred.
• Academic/teaching hospital experience is desirable.
• Experience in research or performance improvement desirable.

Responsibilities

• Oversee clinical pharmacy services for transplant patients (pre-, peri-, and post-transplant).
• Provide direct clinical support and participate in daily rounds to optimize pharmacotherapy.
• Develop and monitor evidence-based protocols, order sets, and guidelines.
• Oversee medication inventory, cost management, and workflow in transplant services.
• Educate providers and trainees; serve as a transplant pharmacist preceptor.
• Lead research and performance improvement projects related to transplant pharmacotherapy.
