Health Here seeks a Solid Organ Transplant Clinical Pharmacy Specialist to provide advanced pharmacotherapy management for transplant patients in inpatient and ambulatory settings. You will optimize immunosuppressive regimens, monitor therapy and interactions, collaborate with multidisciplinary teams, and educate patients and caregivers.

You will precept and mentor learners and contribute to protocol development.
