Job summary

Mayo Clinic in Phoenix, AZ is seeking a Clinical Specialist Pharmacist – Transplant to join our organ transplant team. You will provide expert pharmacotherapy consults for outpatient and inpatient abdominal transplant services and support heart and lung as needed.

Role requires an ACPE-accredited pharmacy degree, residency and board certification in transplant or equivalent, and AZ pharmacy licensure. Competitive benefits and a demanding 80-hour schedule with growth opportunities are offered.

Qualifications

• Graduation from a Pharmacy program accredited by the Accreditation Council for Pharmacy Education.
• PGY2 residency in a specialty area with board certification or PGY1 with 2+ years post-licensure in a specialty with board certification.
• If no residency, 4+ years post-licensure with 50% in specialty and board certification.
• Prior experience in transplant or related specialty preferred.
• Knowledge of Joint Commission, federal and state regulations governing pharmacist practice.
• Excellent communication with patients and other providers.
• Current AZ State Board of Pharmacy license.
• Current, active board certification in area of specialty.

Responsibilities

• Provide expert consultation for solid organ transplant medication use.
• Collaborate with transplant providers for inpatient and outpatient care.
• Optimize transplant outcomes with evidence-based therapies and monitoring.
• Support regulatory requirements and quality programs for safe medication use.
• Provide pharmacokinetic consultations as required.
• Educate transplant team and patients on therapy and adherence.
• Lead and participate in interdisciplinary committees and protocol development.
• Coordinate issues and therapy recommendations with pharmacy staff.
• Support research activities of the transplant team.
• Participate in meetings covering discharge management, research, and teaching.
• Stay updated on transplant protocols and expand transplant service support.
