Work with the transplant team to optimize therapy. Requirements: PharmD or Bachelors in Pharmacy, PGY2 in Solid Organ Transplant or equivalent, 5 years transplant experience, PA license, and board certification within 2 years.

Qualifications

• 5 years experience in pharmacy institutional practice in organ transplant or PGY2 Solid Organ Transplant residency

Responsibilities

• Provide pharmaceutical care to transplant recipients and living donors as part of the transplant team
• Collaborate with physicians, nurses, social workers, and coordinators to optimize therapy

Education

Bachelor's Degree Pharmacy or PharmD
