Job summary

The Clinical Pharmacist Specialist for Solid Organ Transplant is responsible for the provision of pharmaceutical care to abdominal organ transplant recipients and living kidney donors. This role works with the transplant team to optimize therapy, monitor drug interactions, and educate patients and families in a culturally competent manner.

Requires PGY2 in Solid Organ Transplant or equivalent, 5 years hospital transplant experience, and PA license in Pennsylvania.

Qualifications

• Pharmacist with 5 years of hospital/institutional transplant experience or completion of PGY2 in Solid Organ Transplant.
• License to practice in PA required.

Responsibilities

• Provide pharmaceutical care to abdominal organ transplant recipients and living donors.
• Collaborate with transplant team to optimize therapy and monitor drug interactions.
• Educate patients and families and coordinate with physicians, nurses, social workers, and coordinators.
