Job summary

UI Health in Chicago, IL seeks an Advanced Practice Provider (APP) - Transplant Outpatient to join the Transplant department. The APP collaborates with physicians to deliver high-quality, cost-effective outpatient care across all ages, with a focus on transplant medicine.

The role requires NP/PA certification, state licensure, BLS, and ongoing professional development; travel to offsite transplant clinics may be required.

Qualifications

• Graduation from a nationally accredited APRN or PA training program.
• Current National Certification as ACNP-BC/AGACNP-BC or PA-C is required.
• Current Illinois APRN-CNP or PA-C license required.
• BLS certification and DEA registration are required.

Responsibilities

• Works within bylaws and hospital policies.
• Collaborates with the care team to manage patient care.
• Performs comprehensive history and physical examinations.
• Orders and interprets tests and treatments within scope.
• Prescribes medications and care plans per privileges.
• Maintains nursing and physician practice standards.
• Contributes to quality improvement and patient safety.
